How to Navigate Form Question Types


There are 6 different question types you may find within a form:

  1. Inspection Items have a Check Mark, X, N/A selection, and in the event of a failure (selecting ‘X’), you will be prompted to add a ‘Corrective Action’ for the specific hazard.

From the ‘Corrective Action’ screen, you can set it as having been resolved or unresolved:

  • Describe the hazard control and assign it to the responsible person.
  • Rank the Severity & Probability using the scrolling bar:

i. Select the appropriate number for Severity and letter for Probability. You will notice the

description and color coding will change based on the information inputted.

ii. You can also choose the frequency of the hazard.

iii. Add any image or other comments to further support your corrective action.

iv. You can also draw on the image or import from your photo library.

v. Once you have filled in the information, tap Save to return to filling out the form.

  1. Text Fields are simply areas that allow for the individual filling out the form to type a response as per the instructions in the name and description of the question. This question type allows you to add a photo if needed
  2. Checkboxes are non-mandatory questions and only require the individual to check the box pending that the conditions described in the specific question have been met.
  3. Yes/No questions are elements that require the individual to respond with a Yes, No or N/A answer, and can provide comments or a description if desired. This question type allows you to add a photo if needed
  4. Common Hazards have a ü, N/A if a hazard is present, then you will be prompted to the ‘Corrective Action’ screen in the same manner as Inspection items.
  5. Instruction questions are simply text that you have included in the form as an extra note that is not specifically related to any other form question type. Once you have filled in all the information for your question, press the check mark to add it.
